Role Responsibilities
• Delivering safe, effective and compassionate palliative care
• Managing patients with complex healthcare needs
• Supporting patients, carers and partner agencies to maximise quality of life
• Helping reduce avoidable hospital admissions
• Supporting patients to receive dignified end-of-life care in their preferred setting
• Supervising, supporting and educating junior team members
• Acting as a professional role model within the department
• Supporting the Clinical Team Manager in meeting organisational objectives
• Ensuring high standards of clinical care, professionalism and cleanliness
• Communicating sensitively with patients and families regarding care and prognosis
• Promoting NHS values, standards and compassionate care practices
What The Employer Is Looking For
• Registered healthcare professional working within NMC or HPC standards
• Leadership and team supervision experience
• Strong communication and patient care skills
• Experience supporting patients with complex or palliative care needs
• Ability to work collaboratively with multidisciplinary teams
• Compassionate and professional approach to patient care

• The position involves supporting Hospice Day Services operations
• Candidates will help ensure clinical standards and patient safety are maintained
