COMMUNITY SUPPORT WORKER
Employer: Tees Esk and Wear Valleys NHS Foundation Trust
Location: Durham (Inglewood, Lanchester Road Hospital, DH1 5RD)
Salary: £24,937–£26,598 per annum (Band 3)
Contract: Permanent
Working Pattern: Full-Time

ROLE OVERVIEW
• Work within the Community Rehab and Recovery Team
• Deliver high-quality, patient-focused care in community settings
• Support people with mental ill health to promote recovery and wellbeing
• Assist registered practitioners with delegated clinical duties
• Promote social inclusion and community participation
• Accompany patients to appointments and community activities
• Maintain accurate patient records and report changes appropriately
• Participate in quality improvement activities
ESSENTIAL REQUIREMENTS
• Diploma in Health and Social Care Level 2 (or equivalent)
• Care Certificate
• Literacy and Numeracy Level 2
• Previous experience as a Band 2/3 HCA or equivalent
• Effective communication and record-keeping skills
• Commitment to safeguarding and person-centred care
• Completion of Trust HCA competencies within 6-month probation
ADDITIONAL INFORMATION
• Enhanced DBS required
• Right to Work in the UK required
• Starting salary does not meet Skilled Worker visa sponsorship threshold
