Summary
NHS Work Well is recruiting a Health Coach to support patients in improving their health, wellbeing and independence through personalised coaching and self-management support. The role involves managing a caseload and helping individuals develop healthier lifestyles and better long-term condition management.
Key Responsibilities
- Manage and prioritise a patient caseload
- Deliver personalised health coaching sessions
- Support patients to create and achieve wellbeing goals
- Encourage healthy lifestyle changes and self-management
- Support shared decision-making and patient independence
- Deliver group consultations and education sessions
- Signpost and refer patients to appropriate services and support
Requirements
- Strong communication and motivational skills
- Ability to work with people in a non-judgemental and supportive way
- Experience supporting health and wellbeing initiatives preferred
- Ability to manage sensitive conversations professionally
- Teamworking and organisational skills
