About the Role
- Manage a caseload as a Nurse Assessor / Case Officer
- Support patients with complex health needs
- Ensure care packages are safe, effective, and person-centred
Key Responsibilities
- Carry out comprehensive health and needs assessments
- Coordinate and review complex care packages
- Contribute to eligibility decisions
- Work with multidisciplinary teams across health and social care
- Support patients and families with clear communication
- Maintain accurate and compliant records
- Contribute to service improvement and audits
Requirements
- Professional registration (NMC or SWE)
- Postgraduate qualification or equivalent experience
- Valid UK driving licence
- Extensive experience in health or social care
- Experience in care planning, case management, and partnership working
- Strong clinical judgement and communication skills
